2009-2010 WINTER CLUB HOCKEY REGISTRATION

Wednesday September 2^nd, 7:00 p.m.- 9:00 p.m., WC Rink

7 Easy Steps

(complete steps 1-6 below PRIOR to coming to the rink on 9/2)

1. Go to www.usahockeyregistration.com (click on “ice players and coaches”).  Register your child EXACTLY as his/her birth certificate lists his/her name (“legal name”) and DOB. Please make sure to complete ALL information on the USA Hockey Registration form. By doing so will say us a great deal of work. You will need the child specific USA Hockey confirmation number that USA Hockey will email to you in order to continue your registration. Print this form.
2. Complete the Winter Club online registration form HERE. When you have completed the form, hit submit once. You will receive a confirmation page. Print this page.
3. Print and complete the Parent Code of Conduct form FOR EACH SKATER. Click HERE for form.
4. Print and complete the Player Code of Conduct form - FOR EACH SKATER. Click HERE for form.
5. Print and complete the Consent to Treat form- FOR EACH SKATER. Click HERE for form.
6. Fees are to be paid in full at the time of registration.  If you can't make registration you should send all your paperwork and check to Todd Endres at N36W7393 Buchanan Street, Cedarburg, WI 53012. Make your check out to Winter Club.  A late fee of $50 per skater is applied beginning Sept 10.

7. Bring each of the completed 5 forms listed above, as well as your check to registration at the WC Rink on Wednesday September 2^nd from 7:00 p.m. – 9:00 p.m. 

NOTE: If your child is an Atom or a 1^st year Mite or A FIRST YEAR PLAYER AT WINTER CLUB, your must also provide a copy of your child’s proof of age (birth certificate, passport, etc.) at the time of registration.  We cannot process your child’s registration without this documentation.
