2011-2012 WINTER CLUB HOCKEY REGISTRATION 

It's not too late to start youth hockey! Still accepting Atoms registration forms!

8 Easy Steps
(complete steps 1-7 below PRIOR to coming to the rink on 9/7)

1. Go to www.usahockeyregistration.com (click on “ice players and coaches”). Register your child EXACTLY as his/her birth certificate lists his/her name (“legal name”) and DOB. Please make sure to complete ALL information on the USA Hockey Registration form. By doing so will save us a great deal of work. You will need the child specific USA Hockey confirmation number that USA Hockey will email to you in order to continue your registration. Print this form.
2. Complete the Winter Club online registration form HERE. When you have completed the form, hit submit once. You will receive a confirmation page. Print this page.
3. Print and complete the Parent Code of Conduct form FOR EACH SKATER (found below).
4. Print and complete the Player Code of Conduct form - FOR EACH SKATER (found below).
5. Print and complete the Consent to Treat form- FOR EACH SKATER (found below).
6. For new skaters to Winter Club and all Mites, print and complete the Annotation of Birth Facts form- FOR EACH SKATER(found below). Please bring it and an official birth certificate to registration. You do not need to sign the form; it will be signed by Winter Club representatives at registration. If you are mailing your registration in, please mail the original birth certificate. It will be returned to you at the start of the season.
7. Fees are to be paid in full at the time of registration. If you can't make registration you should send all your paperwork and check to Heather Erlandson, 1718 E Fox Lane Fox Point, WI 53217. Make your check out to Winter Club.

A late fee of $50 per skater is applied beginning Sept 14 (Does not apply to Atoms).

No installment payment options will be offered this season. You child will not be able to skate if full payment has not been received before tryouts.

For Pee Wee A and Bantam A teams, there will be an additional $150 fee that will be collected at the start of the season. Base fees for the 2011/2012 year are below. There is no volunteer requirement or credit.

8. Bring the following to the rink on Wednesday September 7th from 7:00 p.m. – 9:00 p.m.:

  • USA Hockey Registration Confirmation
  • Winter Club Registration Confirmation
  • Annotation of Birth Facts
  • Original Birth Certificate
  • Player Code of Conduct
  • Parent Code of Conduct
  • Consent to Treat
  • Check for 2011/2012 base fee
